Northern California Collaborative for PPC (NCCPPC) Relaunch

Dear Northern California Colleagues, 

The newly formed Division of Quality of Life and Pediatric Palliative Care Division at Stanford Medicine Children’s Health is excited to invite you to a Lunch & Learn onThursday, December 5th from 10:00 am - 2:00 pm. 

Our hope in creating this event is to bring together providers as we relaunch the Northern California Pediatric Palliative Care Collaborative. We are eager to connect with fellow professionals dedicated to advancing pediatric palliative and hospice care in our region. As many of us are new to the Bay Area, this meeting presents a valuable opportunity to learn about the remarkable work each of you is doing for our community. Our goal is to enhance and build upon our collaborative efforts to provide comprehensive and compassionate care for children and their families. 

To facilitate meaningful discussion and effective collaboration, we kindly request each provider/program to prepare a max of 6 slides covering:  

• A brief overview of your organization
• Services you provide and your catchment area. 
• Are you on the California map
• Identify any gaps you see in your area
• Your hopes of what you want to see from the coalition and what are you hoping for from this network.
